Carry out a Pre assessment to make sure that we can meet and support the needs of the service users. Once the service user comes into our care and support we will carry out a robust risk assessment and management plan to minimise the risk to keep them safe.
Service users will be supported to access the G.P Surgery and the Secondary Services to make sure that the reviews are carried out regularly.
